Your calculation is wrong:

You have to add 2 x 1N 4148 and a poti to the list...

@ Snail's link above, there's a YouBoob vid showing the pedal - whatever - the cool thing is if you look at this same cat's other videos, there's a few with a hip ol' bluesman of sorts playing what seems to be a 70's Strat w/ 3 'trons or 'buckers of some type - with lots and lots of phase 90 - groovy, baby.
